The real causes of the incidents are never identified and too much focus is put on assigning blame. To make matters worse, little attention is given to how incidents can be avoided in the future.

ABOUT THE SPEAKER: Larry R. Russell is a Senior Consultant and Manager of Internal Consultant Implementations for Behavioral Science Technology, Inc. (BST). He is a Certified Safety Professional (CSP) and a Professional Member of the American Society of Safety Engineers.
Larry has been a safety professional for 30+ years, the last 14 of those with BST.
